- Appointments be booked under your child’s name.
- For kids 13 and under: A parent or guardian must complete the intake form in advance and must attend the first appointment. Please be prepared to stay for the entire duration.
- Your child must consent to receiving care.
- Appointments should be therapeutic in nature (have a reason for treatment).
